Personal Health Audit
Personal Health Audit
A structured way to bring your medical history, results and measurements together, so that you arrive at your next appointment prepared.
Why it exists
When I began investigating my own health, I discovered how difficult it was to assemble a complete picture. My results were scattered across different years, laboratories and doctors. Only when I brought them together did patterns emerge, and with them questions I had not thought to ask.
The Personal Health Audit is the checklist I wish I had started with.
What it helps you organise
- Medical and family history
- Blood-test results, with their dates
- Weight and waist measurements
- Blood pressure
- Medications and supplements
- Nutrition and exercise
- Sleep and lifestyle
- Questions for your doctor
What it is not
The audit does not interpret your results, diagnose a condition or recommend treatment. It is a way of organising your own information so that you and your doctor can make better use of it.
Availability
The first version is being finalised and is not yet available to download. If you would like a copy when it is ready, email me with “Health Audit” in the subject line and I will send it to you. I will use your address for that purpose only.
